Marie Curie Cancer Care is a UK charity dedicated to the care of people with terminal cancer and other illnesses. Over the financial year 2010/11, we reached a total of 31,799 patients

New Patient Registrations
Brunel Medical Practice are please to announce that we are now taking on new NHS patients.
The Partners of Brunel Medical Practice
To register with the practice, you must complete the Brunel Medical Practice New Patient Form and the GMS1 Registration Form. These are available below for you to print off and complete.
Once you have completed and signed the forms, please hand both forms in to the surgery in person.
N.B. you will need to complete both the New Patient form and the GMS1 registration form to enable us to complete your registration.
You will also need to provide proof of identity to register. This can be TWO forms of photographic ID (eg passport/driving licence/bus pass/armed forces ID card etc) OR ONE form of photographic ID PLUS a bank statement (not a copy). Please note that no copies will be made of bank statements and these are used purely to cross-check against photographic evidence.

These fact sheets have been written to explain the role of UK health services, the National Health Service (NHS), to newly-arrived individuals seeking asylum. They cover issues such as the role of GPs, their function as gatekeepers to the health services, how to register and how to access emergency services.
Special care has been taken to ensure that information is given in clear language, and the content and style has been tested with user groups.

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Registration Letters from NHS England

The NHS is carrying out work in Devon, Cornwall and Isles of Scilly to make sure that the patient details on GP lists are accurate. Letters are being sent to some patients, but not all.
If you do receive a letter, please either confirm your details or submit any changes. Changes can be made by returning the form in the prepaid envelope. If the details are correct, you can confirm by freepost, by freephone, via text message or online.
It is important to respond to the letter, as GP lists do get out of date over time. If we don't have an accurate record of your name and address, then we or a hospital may not be able to contact you with important information.

With test results
To invite you for bowel, breast or cervical screening to protect against cancer for example
To offer vaccination against infectious diseases such as flu or measles, mumps and rubella
To give you an appointment as part of your plan to stay healthy if you have a long term illness
Please respond to the letter within four weeks, so the NHS can be certain about your details. Otherwise, the NHS will assume that you have moved away, and begin the process to remove you from the practice list.
Named Accountable GP
From 1st April 2015 all Brunel patients, including children, have a Named Accountable GP responsible for their care at the surgery. This does not change which doctor you can see.
If you would like to know who your named GP is, please ask at reception.
